I was at work and a Technician asked me to remove a component that was powered by a variac variable transformer plugged into 110v ac. I told him to unplug the equipment. He dialed the variac to zero volts. I insisted that he unplug it. He threw the switch double throw double pole and he asked me to remove the component. I probed the ‘turned off’ circuit to ground and it read 110v ac. I unplugged it and probed the vriac and found the switch was arc-welded closed and the variac was wired in correctly. It was wired to bring the neutral up to the line voltage instead of bringing the line down to neutral.
This was a professional lab at a major defense contractor in the 80’s. Now there are procedure’s and training to follow electrical safety. It is hard to imagine how this piece of equipment passed calibration with those two obvious defects.
When I was 10 I noticed a slight shock when I touched a toaster while leaning on a stainless range. I picked up a 100 watt light bulb and a knife to connect the light bulb base to the range and tap the toaster case to the center terminal of the light bulb. It lit up full intensity! Upon investigation, the toaster didn’t have a wide blade to key it to neutral or polarize the plug. But one wire was tied to the case as if it were neutral. This was a bank gift for opening an account with 500 dollars. It was obviously a refurbished toaster where the polarized plug was replaced with one that wasn’t polarized. You have a 50/50 chance of a disaster but a 100% hazard.
